Abnormal Gastric Myoelectrical Activity and Delayed Gastric Emptying in Asthmatics: What Is Clinical Significance?

چکیده
TO THE EDITOR: I read with interest the study by Amarasiri et al regarding the relationship between gastric dysmotility and asthma using electrogastrography (EGG) and real time ultrasonography. The study was conducted by enrolling 30 asthmatics and 30 healthy volunteers. The study reported that asthmatics showed abnormal gastric myoelectrical activity (GMEA), delayed gastric emptying (GE) and antral hypomotility in response to a solid meal, compared to healthy controls. Abnormal GMEA especially during the postprandial period is associated with severe gastroesophageal reflux disease (GERD) or delayed GE. Delayed GE contributes to a progressive dilatation of the proximal stomach which, in turn, shortens the length of the lower esophageal sphincter until it becomes competent. This is why delayed GE can promote gastroesophageal reflux episodes, and then can provoke the exacerbation in asthmatics. I would like to consider 2 concerns for the accurate understanding of the meaning of these results. First, the clinical significance of abnormal GMEA was not clear. It must be noted that EGG does not diagnose a specific disease, even the stomach motor dysfunctions. The ranges of % normal waves, dominant frequency, pre/postprandial power ratio were also within normal range, despite of the significant differences between the 2 groups. However, the normal range of GMEA observed in asthmatics should not be devaluated because GE is the final coordination of fundus, antrum, pylorus and duodenum, and then normal EGG parameters do not guarantee normal GE. Finally, delayed GE observed in asthmatics group might have resulted from the high proportion of GERD among asthmatics. In other words GERD might be confounding factor between the delayed GE and asthma. This is because delayed GE is frequent in patients with GERD. Further larger study with well-designed methodology is needed to confirm whether abnormal GMEA, delayed GE and antral hypomotility are significantly more prevalent in asthmatics.

Gastric Motility Following Ingestion of a Solid Meal in a Cohort of Adult Asthmatics
BACKGROUND/AIMS Asthmatics have abnormal esophageal motility and increased prevalence of gastroesophageal reflux disease (GERD). The contribution of gastric motility is less studied. We studied gastric myoelectrical activity, gastric emptying (GE) and their association with GERD symptoms and vagal function in adult asthmatics. METHODS Thirty mild, stable asthmatics and 30 healthy controls und...
متن کاملElectrogastrographic study of gastric myoelectrical activity in patients with unexplained nausea and vomiting.
Using cutaneous electrodes an electrogastrographic study was made of gastric myoelectrical activity in both the fasting and postprandial states in 48 patients with unexplained nausea and vomiting and in 52 control subjects. A gastric emptying study, using a radio-labelled solid phase meal, was carried out in 30 of these 48 patients. A follow up study was done after one year. In 48% of the patie...

Introduction: Gastric drainage disorder is one of the complications of gastric pull-up and esophagectomy after surgery which might lead to esophageal cancer and benign strictures. The aim of this study was to determine the role of pyloromyotomy on gastric drainage. Materials and Methods: In this prospective randomized controlled clinical trial study, we studied 51 patients in two matched groups...

Experiments were designed to study the effect of proximal gastric vagotomy (PGV) followed by total vagotomy (TV) on the myoelectrical activity of the canine stomach and duodenum after a meat meal and during fasting. Dogs were prepared with chronically implanted Ag-AgCl-electrodes on the stomach and duodenum. Recordings of electrical activity were made for one hour after the ingestion of a meat ...

It is widely accepted that the inhibition of gastric motor activity as well as the maintenance of gastric mucosal blood flow and mucous secretion are important for the homeostasis of the gastric mucosa. The present study was performed to ascertain whether or not endogenous PGs, which can protect the stomach from noxious stimuli, affect gastric motor activity and emptying. The myoelectrical acti...
